Caused by the sheet being misfed after the lithograph print was applied. Then before applying the steeled print, the sheet was flipped. Instead of being printed on top of the litho on the front, it was applied over the image on the back side of the note. Then the serial number was printed on the wrong side as well.  Note the bar code, this proves the they are applied at a different time as the serial numbers.
